The manager has set up a great system at Tony Marbell's Cantina. He has a full page where he asks for name, address, email, birthdays (does a special with that) and other questions regarding how they thought service was. But the wait staff personally gives them out to each customer. In return for filling them out, they are entered for the monthly drawing for a $25 gift certificate. He's seen great response from that and he's able to use the info for follow up.
